If you would like to request a school bus for your fourth or fifth grade child that is attending the Academy (76-05 51st Avenue, Elmhurst NY 11373), please fill out the school bus exception form.
Important! When selecting the school from the drop down menu, please select the correct location: "24Q007 - P.S. 007 Louis F. Simeone - 51ST AVENUE ACADEMY - QUEENS" (see attached screenshot).
Please note:
- Students who do not qualify for busing based on grade and distance should apply for an exception by creating a ticket via the link.
- The bus exception form must be completed each school year. Previous school bus requests do not roll over into the new school year.
