If you would like to request a school bus for your fourth or fifth grade child that is attending the Academy (76-05 51st Avenue, Elmhurst NY 11373), please fill out the school bus exception form.
Important! When selecting the school from the drop down menu, please select the correct location: "24Q007 - P.S. 007 Louis F. Simeone - 51ST AVENUE ACADEMY - QUEENS" (see attached screenshot).
In the “Additional Information” section, select:
- School Year: 2024-2025
- School Term: Fall (Sept-Jun)
- Reason: “There is a hazard (danger or risk) that makes getting to the school unsafe”
- Borough of Hazard: Queens
- Intersection/Street 1: Queens Boulevard and 51st Avenue
- Choose: “Busy street but no stop sign, traffic light, and/or pedestrian crossing”
- Intersection/Street 2: Put the intersection of your residence
Please note:
- Students who do not qualify for busing based on grade and distance should apply for an exception by creating a ticket via the link.
- The bus exception form must be completed each school year. Previous school bus requests do not roll over into the new school year.
